Arranging A Free Sperm Extraction Consultation

Please provide your name, address, what IVF doctor you are working with (if any), and in the subject line, please put:
"Sperm Extraction Consultation Request"
Dr. Russell's office will then mail you a packet which contains a questionnaire which reviews some general health and fertility questions for you and your wife. Please complete the questionnaire and mail it back to Dr. Russell's office in the included pre-stamped return envelope. When Dr. Russell receives the completed questionnaire, he will contact you to review your individual fertility situation and answer your questions.
When Dr. Russell receives your email request for a consultation, he will send back a confirmation email. As we all know, sometimes emails/letters get lost en route. If you have not received a confirmation email within a few days of your sending the request, please either re-send your email request or call Dr. Russell's Dayton office at 937-434-6344 and leave a message requesting a phone consultation packet. Please also use this contact information if you have not heard back from Dr. Russell within a week of mailing back your questionnaire.
What if I prefer a regular office consultation?
If you live in the Dayton/Cincinnati/Columbus region, Dr. Russell is available to see you in his office in these cities if this is preferred over an initial phone consultation.
