Once you submit the on-line form on this page, Dr. Russell's office will mail you a packet which contains a questionnaire that reviews some general health and fertility questions for you and your wife/female partner. Please complete the questionnaire and mail it back to Dr. Russell's office. When Dr. Russell receives the completed questionnaire, he will contact you to review your individual fertility situation and answer your questions regarding the reversal procedure.
If you would prefer to have a phone consultation before requesting a questionnaire be sent, just use the on-line form but only send your email address and contact phone number (not mailing address). Dr. Russell will then contact you to review your options in detail and answer your questions.
What if I prefer a regular office consultation?
